I have been contacted by an outside source, through iStock, to do custom work for them. What are the rules for this?

Just do the work and bill them as you would any freelance job, it's nothing to do with the site.

Like John said, this would be 'work for hire', so any agreement you have with the client will need to be outside of iStock. The exception is if you would like to sell your illustrations on iStock after: In that case, you'll need to state this in your contract with the client. Hope this helps!
