I have photos from 2008 when my son was born via caesarean section. I will submit a model release to be safe (although I don't think anyone, including the model, my son, would be able to identify him in the photo). The witness and I would sign with a current date with the shoot date on my son's birthday in 2008...is this correct?
Yes, the shoot date field is the day you actually took the photo. The date you (in the photographer & the parent sections) and the witness actually sign the model release is the date you write in the the Date Signed field.
